St. Joseph Medical Center’s CEO looks to the future

TOWSON – Late last year, St. Joseph Medical Center agreed to pay the U.S. government $22 million to settle allegations it was involved in a kickback scheme with MidAtlantic Cardiovascular Associates (MACVA). Dr. Mark Midei, a one-time MACVA partner who later worked for St. Joseph, was accused of performing medically unnecessary stent procedures.

In the wake of the federal settlement, Jeff Norman, St. Joseph’s new president/CEO, has overseen an extensive overhaul of some of the hospital’s policies.

But he noticed a tightness in his chest sometimes. So he went to St. Joseph Medical Center in Towson and saw Dr. Mark G. Midei, who found significant blockage in three major coronary arteries and used mesh stents to prop open two of them.


Johnson says he has been irreversibly disabled ever since. According to a lawsuit filed this year in Baltimore County Circuit Court, the stents caused a blood clot, which led to a heart attack and a series of medical events that left Johnson with permanent heart damage.  Click here to read more of the Baltimore Sun Article.

Three hospitals have consistently outpaced all others in Maryland in the use of stents to treat heart patients, according to data obtained by The Baltimore Sun, raising questions as state regulators examine whether the expensive procedure is performed unnecessarily by some doctors.

St. Joseph Medical Center in Towson became the subject of a state investigation earlier this year after it notified 585 patients that they might have received unneeded stents to prop open their arteries. The new data, from the Health Services Cost Review Commission, shows that Union Memorial Hospital in Baltimore and Washington Adventist Hospital in Takoma Park are also likely to be subject to review.


Each of the three hospitals exceeded the state's average rate of placing a stent after diagnosis by 20 percent to 30 percent over the past five years. State health officials investigating claims that a Towson cardiologist performed hundreds of unnecessary medical procedures have identified other doctors in Maryland with similarly questionable practices, The Baltimore Sun has learned.

Several cardiologists in the state have performed a suspiciously high number of the same invasive cardiac stent procedures that Dr. Mark G. Midei is accused of over-performing at St. Joseph Medical Center, according to an analysis of data from the state's Health Services Cost Review Commission.

The discovery was made as part of a broader statewide investigation of hospitals that was triggered by the allegations at St. Joseph, and is raising concerns that the accusations against Midei might not be isolated.

Investigators plan to review the work of the other cardiologists — who were not identified to The Baltimore Sun — going back five years. Sources also said they expect the investigation to eventually expand to include inquiries into other medical specialties and procedures.

Del. Peter A. Hammen, who asked the state agencies to investigate, cautioned against jumping to conclusions about what the higher rates of stent placement indicate.

"Once we have [all] the numbers in front of us, we'll be able to make, obviously, better sense as to what they mean and what actions we should take," said Hammen, who was briefed on the initial data. "It's early in the state's investigation."

State investigators, after interviewing medical specialists and analyzing billing data reported by hospitals, have determined that doctors who perform a diagnostic test called a cardiac catheterization typically find blockage requiring treatment with a stent about 30 percent of the time.  Click to read more.

St. Joseph Medical Center in Towson, whose cardiology business was a focus of a federal health-care fraud investigation, notified hundreds of its heart patients that they may have received expensive and potentially dangerous coronary implants they didn't need.

An internal review, begun last May at the behest of federal investigators and in response to a patient complaint, turned up 369 patients with stents that appear to have been implanted in their arteries unnecessarily, CEO Jeffrey K. Norman said. Patients began receiving letters alerting them to the finding in December 2009, and more notifications were expected."We take our interaction and the care of our patients with the utmost seriousness, and so we wanted to alert patients and their physicians to what we found," said Norman.  Click to read more.

The story Frank Ward tells about his medical history would sound familiar to just about any malpractice attorney in Baltimore.

He went to St. Joseph Medical Center for a heart procedure, Dr. Mark Midei put a stent in his artery after finding a blockage, and now another doctor says the blockage was insignificant and the stent wasn't necessary. A total of 585 patients have received letters from St. Joseph describing similar scenarios, after the Towson hospital reviewed the last two years of Midei's cases.

"I feel betrayed," said Ward, a 50-year-old courtroom constable from Perry Hall.

But similarities aside, Ward's case also represents a whole new category of potential problems for St. Joseph, and it suggests that the hospital could be facing legal claims from many more of its former patients.
